Preparticipation Physical Evaluation Medical Eligibility Form
All athletes are required to complete the following forms and return to the school nurse's office for any athletic participation:- Preparticipation Physical Evaluation Medical Eligibility Form is on file with the school nurse and dated within 365 days prior to the first practice.
- The school physician must review and clear all medical paperwork prior to clearance to participate!
- Any Extra Forms required are on file with the school nurse (Asthma Action Plan, Medication Forms, etc.)
Health History & Physical Paperwork
The following forms are the up-to-date state medical forms that are to be kept on file at your primary doctor's office:
- Sports Physical Form- The form must be dated within 365 days of the first day of practice. An updated Sports Physical MUST be on file in the Health Office for each athlete. Both pages of the physical MUST be completed by the physician. If your physician does not fill out all information (ex: for vision, 20/__ R 20/__ L), we must reject your physical form.

- Health History Update Form- Must be completed for EACH sporting season and be dated within 90 days of the start of each sport. If your last sports physical was performed within 365 of the first day of practice you only need to fill out the History form.

No faxes, scans, or e-mails will be accepted!! DO NOT hand paperwork to the coach!!
